Club Member Forms
To help us ensure a safe environment for our in-person events, parents/ guardians of club members must fill out the MYBC permission forms either by printing and sending a copy OR by filling out these forms online via the link below. There is no cost associated with these forms, joining our club, or participating in our activities.
